EXPLANATION OF MEMBER FAQs
If you enrolled in a UCare Individual or Family Plan (i.e., purchased on MNsure) for 2026 or are enrolled in a Medical Assistance (Medicaid) Plan for 2026, your coverage for 2026 is NOT impacted in any way by the Rehabilitation.
MEMBER CLAIMS RELATED TO MEDICAL SERVICES RECEIVED IN 2025:
The Rehabilitation Plan (“Plan”) was approved on April 10, 2026, to facilitate the eventual liquidation of UCare and is available for review at this LINK. To the extent there is a conflict or inconsistency between orders of the Court, the Plan and/or this website and any UCare documents, the orders of the Court, the Plan, and this website control.
In general, your claim will be paid in the order in which it was adjudicated. That means that all medical claims will be organized by claim adjudication date with the oldest paid first as cash becomes available to make payments.
The Rehabilitator paid ~$350,000,000 in claims between April 23 and May 11, 2026. The Rehabilitator anticipates paying ~$115,000,000 in claims beginning the second week in July. All claims will not be paid before 2027.
A substantial part of the UCare assets are receivables which are payable to UCare in the future and cannot be accelerated. These assets, when received, will be available to the Rehabilitator to pay claims.
Yes. The Rehabilitator will review the ability of UCare to make additional payments monthly under the proposed Plan of Rehabilitation..
Because UCare in rehabilitation can only pay claims as funds become available, there is a delay in claims payments. As claims are released and paid, explanation of benefits and other documents explaining the adjudication of a member’s claim are released. To the extent you received medical services in 2025 and have not yet received an explanation on the adjudication of those services, we are hopeful that you will receive that explanation by the end of the first quarter of 2027.
Yes. June 30, 2026.
If you have a claim for something other than medical services, please file that claim with UCare prior to June 30, 2026 or you may file a Proof of Claim with the Rehabilitator using this [LINK]. Please do not file the claim with UCare and through the Proof of Claim process, as this will delay adjudication of your claim. A determination will be made on the amount, if any, owed on that claim. You will receive notification of the amount allowed and that amount will be deemed a Priority 5 claim in the UCare rehabilitation proceedings. Priority Class 5 payments will be deferred until all Priority Class 1 through 4 claims are paid in full or sufficient cash is reserved to ensure full payment of these higher priority claims.
Generally, no. Providers are prohibited from balance billing members for covered medical services. For more information, please visit https://www.ucare.org/legal-notices/no-surprises-act.

EXPLANATION OF PROVIDER FAQs
The Provider FAQs below are organized in two sections. The first section is applicable to all claims for dates of service on or before December 31, 2025. These claims are subject to the Rehabilitation proceeding described above and the FAQs in the first section relate only to those claims. The second section is applicable to all claims for dates of service on or after January 1, 2026. These claims are not impacted by the Rehabilitation proceeding.
CLARIFYING NOTE: We are aware of confusion regarding whether claims for dates of service on or after January 1, 2026, will be paid. All claims for dates of service on or after January 1, 2026, will be paid in the ordinary course.
SECTION 1 – DATES OF SERVICE 12/31/2025 AND PRIOR
PROVIDER CLAIMS FROM 2026 ARE EXCLUDED FROM THIS REHABILITATION PLAN.
2026 PROVIDER UCHP CLAIMS ARE BEING PAID ON A REGULAR CALENDAR SCHEDULE.
The Rehabilitation Plan (“Plan”) was approved on April 10, 2026, to facilitate the eventual liquidation of UCare and is available for review at this LINK.
Some provisions of the Provider Manual have been superseded by the Plan. For example, the requirements around timely filing of claims and appeals have been amended. To the extent there is a conflict or inconsistency between orders of the Court, the Plan, and/or this website and the Provider Manual or any other UCare documents, the orders of the Court, the Plan, and this website control.
Yes. Covered medical services claims under UCare policies will be accepted for all dates of service through December 31, 2025, as long as they are filed by June 30, 2026.
Yes. June 30, 2026 is the deadline for the submission of all provider claims for dates of service on or before December 31, 2025, for which UCare is the primary payer.
Exceptions:
- The deadline for claims for inpatient hospitalization which started during plan year 2025 and continued into plan year 2026 shall be filed by the later of June 30, 2026, or within three (3) months of the patient discharge.
- For claims for which UCare is not the primary payer, the deadline for submission is the earlier of three (3) months after the date on which the provider of health care goods or services is notified that UCare will have primary responsibility for the claim or December 31, 2026.
Payment of provider claims filed after the Rehabilitation Plan’s filing deadlines prescribed by the Plan will be delayed until UCare is ordered into a formal liquidation proceeding and shall be considered Priority Class 4 claims and adjudicated by the Liquidator accordingly.
Yes. For claims released in 2026, you have three (3) months following the issuance of an Explanation of Payment/Provider Remittance Advice to file an appeal or reconsideration of adverse benefit determinations or payment disputes. For claims released prior to 2026, you have the lesser of 12 months from the issuance of an Explanation of Payment/Provider Remittance Advice or three (3) months from approval of the Rehabilitation Plan to file an appeal or reconsideration of adverse benefit determinations or payment disputes.
There is no change to the existing claims filing process. Current systems, processes and procedures for claim intake will continue.
The reimbursement rates specified in provider contracts have not been amended. The time for filling claims and for filing an appeal or reconsideration of adverse benefit determinations or payment disputes has changed. Please see FAQs 2 and 3 above.
Yes. Pursuant to the Rehabilitation Plan, claims are being released in the order in which they were adjudicated. We have been authorized to release $350 million in claims payments and that process is expected to be completed in mid-May. We anticipate that over one million claims will be released. It is also our intent to release all $0 pay claims as soon as practical irrespective of adjudication date.
We anticipate current liquid assets will be increased by receivables due to UCare over the course of the year. Each month, the Rehabilitator will review whether UCare can make further payments.
No. Our team is working diligently to get claims processed and paid as quickly as possible. Calls for claim status delay the process.
No. Both the Order of Rehabilitation and Minnesota law prohibit balanced billing by all providers (in or out of network) for all claims associated with dates of service on or before December 31, 2025.
The order of Rehabilitation required the Rehabilitator to file a plan of rehabilitation, as described in Minn. Stat. § 60B.17, subd. 8, for the Court’s approval. Section 60B.17, subdivision 8, requires the Rehabilitator to submit a plan that provides for payment in full of each prior class of claims before payment of claims in the next class, and treats all claims of the same class in a fair and equitable manner. The classes of claims and relative priority of each class of claims are set forth in Minn. Stat. § 60B.191, subd. 1.
The Court approved the Plan of Rehabilitation on April 10, 2026.
Claims by all providers for health care goods and services, to the extent covered under an HMO contract, are Priority Class 4 (see Minn. Stat. § 60B.191, subd. 1(4)).
Any provider claims that are not for health care goods and services covered under an HMO contract are Priority Class 5 claims (see Minn. Stat. § 60B.191, subd. 1(5)). For example, interest on claims payments is a Priority Class 5 claim. Payment of Priority Class 5 claims will be deferred until all Priority Class 1 through 4 claims are paid in full or sufficient cash is reserved to ensure full payment of these higher priority claims.
We anticipate posting the Proof of Claim Process for Priority Class 5 and 6 claims on or about June 1, 2026.
Any interest due on claims pursuant to Minn. Stat. §§ 62Q.75 and 256.9655, subd. 3, or as otherwise required by federal law or contract, is a Class 5 claim.
Please email your questions to UCareSDR@examresources.net. We will review all questions submitted and publish additional frequently asked questions identified in the submissions.
If there is a recoupment on an Explanation of Payment and you dispute the recoupment, you may challenge the recoupment by filing your dispute with UCare. To dispute your recoupment with UCare, you must, within 3 months of receiving the Explanation of Payment, upload a Provider Recoupment/Set Off Dispute Form (template form located at ucare-inreceivership.com) explaining that you dispute the existence or amount of the set off.
Disputes must be uploaded at ucare-inreceivership.com, which is HIPAA compliant. Within 60 days of receipt of your completed form, we will advise you of our position on the set off.
To continue with this process and to upload your dispute, click this LINK.
We anticipate most disputes can be successfully resolved by filing your dispute with UCare using the process described above. If you disagree with the UCare resolution, or if you do not wish to first pursue resolution of your dispute with UCare using the process described above, the dispute shall be resolved by filing the dispute with the Rehabilitation Court for its determination or the determination of a Special Master designated by the Rehabilitation Court to adjudicate such disputes.
At this point, Providers should not file a Proof of Claim.
Claims by all providers for health care goods and services, to the extent covered under an HMO contract, are Priority Class 4 (see Minn. Stat. § 60B.191, subd. 1(4)). These claims should be submitted to UCare using the existing claims filing process.
The Rehabilitation and UCare team will work with you to resolve your open A/R issues related to the provision of health care goods and services covered by an HMO contract. If there are issues which have not been resolved as liquidation approaches, we will post a Proof of Claim process for unresolved claims related the provision of health care goods and services.
Interest owed for clean claims, but not paid timely within the time frames required by relevant law, are Priority Class 5 claims (see Minn. Stat. § 60B.191, subd. 1(5)). The Rehabilitation and UCare team are calculating and tracking the interest owed and will provide you with a Deemed Proof of Claim on Interest after the final remit for health care good and services is made. You will have 60 days to dispute the interest calculation. If you opt into the Electronic Notice Process described below, we will email you your cumulative interest calculations as remits are made.
Amounts owed under Value Added contracts are also Priority Class 5 claims. With respect to amounts owed to you or by you under Value Added contracts, we anticipate being able to provide you a Deemed Proof of Claim or Demand for Payment on those amounts by the end of 2026. You will have 60 days to dispute your Deemed Proof of Claim. If there are issues regarding the Demand for Payment that have not been resolved as liquidation approaches, we will post a Proof of Claim process for unresolved claims related to Value Added contracts.
To preserve estate assets and ensure that your Deemed Proof(s) of Claim or Demand for Payment are delivered to the appropriate person(s), we urge you to opt into the Provider Electronic Notice Process described below.
A Deemed Proof of Claim is a Proof of Claim issued to you by the UCare Rehabilitator/Special Deputy Commissioner. The Deemed Proof of Claim will be based on the books and records of UCare. If you disagree with the amount of the Deemed Proof of Claim, you will have the opportunity to dispute the amount.
The Provider Electronic Notice Process is a voluntary process whereby Providers agree to receive notices required by the Rehabilitation process via email instead of U. S. Mail. By opting into this process, you will also receive emails with other pertinent information as it becomes available. For example, we intend to use this process to alert Providers when we will be making future claims payments, releasing claims with $0 owing, and, eventually, releasing all adjudicated claims without remitting the amounts owed. This process will preserve Estate resources by reducing the expenses associated with communicating with Providers, and Providers will receive information more quickly. To opt into the Electronic Notice Process, click this LINK.
Value Based Agreements are a contractual obligation of UCare that will be handled through the Priority Class 5 Proof of Claim process. Any amount owed under Value Based Agreements are not owed pursuant to a contract of coverage and are not for health care goods and services covered under an HMO contract; hence, they cannot be paid at Priority Class 4.
